Based on observations, the facility failed to provide readily accessible exit access as defined by NFPA 101 section 7.2.1.5.10.1.This deficiency affected two (2) of four (4) smoke compartments in the facility on the day of survey.
Findings Include:
On October 17, 2017 between 11:00 AM and 12:00 PM, observation revealed the following doors of the facility contain a locking device 48 inches from the finished floor:
1. Kitchen
2. Housekeeping Room across from the Kitchen,
3. Exam Room 2
4. Patient Room 39

Based on observation and interviews, the facility failed to properly maintain exit egress as per NFPA 101 section 19.2.2.25.2. These deficiencies practice affected two (2) of six (6) exits in the facility on the day of survey.
Findings include:
On October 17, 2017 between 11:00 AM and 12:30 PM, observation revealed the exit door from the Service Hall and the exit door near Patient Room 40 were equipped with a lock that required a key from the egress side.
Interview with the staff revealed no one had key access to these exit doors which creates potential obstruction and lack of discharge to the exit door from the Service Hall and the exit door near Patient Room 40 of the facility.

Based on observations, the facility failed to properly protect hazardous areas in accordance NFPA 101 section 19.3.2.1. This deficiency affected two (2) of four (4) smoke compartments in the facility on day of survey.
Findings Include:
During the building inspection on October 17, 2017at 10:35 AM, observation revealed the doors to Soil Linen Room (near Patient Room 24) and the Data Processing Room lacked an automatic closing devices (door closer). These room were being used as storage and were over 50 square feet in area.
