Based on observation, the facility failed to maintain their corridor doors as evidenced by resident room doors that failed to close and positive latch during testing. This failure affected 1 of 2 smoke compartments and had the potential to allow the spread of smoke from resident room to the rest of the compartment in the event of a fire.
Findings:
During a tour of the facility with facility staff on November 7, 2012, the corridor doors were tested for positive latching.
1. At 10:45 a.m., in the shower room of W-1 Wing, a linen cart was placed in front of a door with a self closing device preventing the door from self closing.
2. At 11:10 a.m., door 244A in Unit -B did not positive latch when closed.
3. At 12:47 p.m., doors 224 and 219 in Unit B did not positive latch when closed.
