Based on observation and interview, the facility failed to provide safekeeping for blood products stored at the facility.
Findings:
Observations on 1/25/2010 at 10:00 AM reveal that the door to the area containing the blood bank refrigerator was unlocked and open to the hallway. Further investigation revealed that the refrigerator was unlocked and that opening it elicited no response from staff.
Interview with the lab director on 1/26/2010 at 3:30 PM reveals that the door to the room can be locked and provides security for the blood products stored in the refrigerator. She further stated that the refrigerator has an alarm. She did not know why the door was unlocked, the refrigerator was unattended, or why the alarm went unanswered.

Based on observation and interview, the facility failed to maintain a safe and sanitary environment by failing to ensure that doors leading to the soiled holding room on the nursing unit are secured.
Findings:
Multiple observations on 1/25/2010 through 1/27/2010 reveal that the doors to the soiled holding room on the nursing unit are not secured and that entrance to the room is through either of two unlocked doors that open to the hallway. The room contained potentially contaminated equipment as well as a box for the disposal of infectious waste containers removed from patients' rooms.
During interview with the administrator on 1/27/2010 at 3:30 PM, she stated that the facility was exploring methods of securing the soiled holding room.
