Based on observation and interview the facility failed to ensure that 1 combustible storage location is equipped with an automatic door closing device.
Findings include:
Observation during tour on 06/01/17 between 10:30 a.m. and 11:00 a.m. with Staff A (Life Safety Coordinator) and Staff B (Director of Facilities) revealed that 1 room, on the lower level (labeled the "Dirt Cellar") is being used to store a large number of boxes containing paper records. The entrance door into the storage area failed to be equipped with an automatic door closing device.
Interview with Staff A and Staff B confirmed the finding and location.
