As a result of the verification visit on 9/10/2012, Surveyor 12316 observed that Item 3 of K-67 tag was not corrected.

The 3rd Floor NW Mechanical Room that was used as a plenum for air returning to the air-handling unit was used to store combustibles that were not completely removed. The combustibles included plywood packing of panels, and data cables and low voltage wiring. The air-handling unit plenum is not allowed to be used for storage or occupancy other than during equipment servicing in accordance with NFPA 90A (1999) 2-3.10.5.2.

Other four items: K-67 1, K-67 2, K-67 5 and K-67 6 have completion dates of 7/19/13, 7/19/13, 12/31/12, 12/31/12 respecrively. The facility has made a satisfactory progress toward correcting the deficiency.

The item K-67 4 has been corrected and verified.

As a result of the verification visit on 9/10/2012, Surveyor 12316 observed that the following K-147 deficiency was not corrected.

The red-colored, critical branch electrical wall outlets in all CVICU patient rooms located on 4th Floor West Hospital were not identified as to which electrical panel and branch circuit the power is supplied from.

Deficiency items K-147 1 and K-147 5 have completion dates of 12/30/2012. The facility has made a satisfactory progress toward correcting the deficiency.

Five other items K-147 2, 3, 4, 6 and 7 on p. 63 - 65 of SOD with Event ID FZYP21 have been corrected.
