Based on review of the facility schematic, observation, staff interview and confirmation the facility failed to ensure the building construction type and height met the requirements of 19.1.6.2 in regards to an unprotected steel roof decking. The facility capacity is 25 with a census of 16 at the time of the survey.
Findings include:
On 3/19/15 between 9:00 AM and 3:00 PM tour of the second floor was conducted with Staff A. Review of the building permit indicated a two story facility with construction type I(332). The second floor of the facility was not provided with an automatic sprinkler system.
During the inspection above the one hour rated ceiling tiles on 03/19/15 at approximately 2:30 PM in the patient waiting area unprotected steel decking was observed. In regard to this observation, the building construction type was determined to be II(000). This was confirmed by Staff A at the time of the observation.
