Observations and interviews with the Certified Dietary Manager (CDM) revealed the Critical Access Hospital (CAH) failed to ensure the premises were clean and in good repair pertaining to the kitchen. The CAH is licensed for 14 beds and had a census of 1 upon survey entrance. Findings are:
A. Observation and interview with the CDM on 4/21/10 at 11:00 AM revealed the following:
1. Observation of the 6-burner, single oven gas stove revealed the insides of the oven and baking rack to be rusted. The outer areas of the 6-burner gas stove along the top ledge, front, and sides were extensively worn and lacking the manufacturer's sealant exposing the underlying metal. The CDM estimated the gas stove to be 30 years old and is used to prepare 3 meals a day for the local jail and hospital patients.
2. Along the South wall of the kitchen are 2 double-paned windows with the inside and outside of the windows with cobwebs and a buildup of dirt and a greasy filmy appearance. CDM stated "not sure when the windows were last cleaned."
3. The individual ceiling tiles located in the kitchen, measuring approximately 2 feet by 3 feet each and cover approximately 15 feet by 35 feet of the ceiling are smeared with a darkened substance identified by the CDM as "dirt smudges and hand prints left by the maintenance personnel when they put up the new ceiling tiles."
4. The following kitchen items were identified to be heavily dented and lacking the manufacturer's sealant, exposing the underlying metal:
- 3 aluminum round 9 inch cake pans
- 2 large aluminum roasters 16 inches by 11 inches
- 2 biscuit warmers
- 5 rectangle aluminum cookie sheets 10 inches by 15 inches
- 2-12 aluminum muffin pans and 1-6 muffin pans
- 1 aluminum angel food cake pan
- 2-12 inch Teflon skillets
- 2-4 quart aluminum colanders
- 1-4 quart wire strainer
- 1 small aluminum funnel
- 1- 8 inch aluminum baking pan
- 3 aluminum 9 inch bread baking pans
- 1 aluminum 8 inches by 10 inches baking pan
- 4-9 inches by 13 inches aluminum baking pans
- 1-18 inches by 10 inches aluminum baking pan
- 1-9 quart porcelain cooking pot
- 1-12 quart porcelain cooking pot
- 1-12 inches by 10 inches aluminum oval roaster
- 1-8 inches by 10 inches aluminum wire rack
- 1-5 quart aluminum teapot
- 2 plastic dish pans
5. The tile flooring, measuring approximately 12 feet by 15 feet located in the xray room has a discolored appearance, is heavily gouged and chipped. Interview with the xray tech revealed the floor has been patched several times, but is probably the original tile.
