Based on surveyor review of the facility's maintenance records on 2/24/16 at approximately 11:00AM, the facility failed to maintain the fire sprinkler system in accordance with Section 19.7.6, 9.7.5, 4.6.12, NFPA 13 & NFPA 25.
The facility's maintenance supervisor was unable to produce documentation that the fire sprinkler system was inspected and tested quarterly and monthly.
