PATIENT RIGHTS: NOTICE OF RIGHTS

Based on medical record review and staff interview, the facility failed to provide four (#2, #3, #4, #9) of 10 sampled closed patient records with orientation on patient rights. This practice does not ensure patient are aware of their rights.

Findings include:

Medical record review revealed that patients #2, #3, #4 and #9 had no documented evidence in their medical record of being informed of their patient rights.

An interview was conducted with the Director of Risk Management at approximately 1:30 p.m. on 10/4/11 in regards to the missing information. She stated that all patients are provided with the Patient Bill of Rights upon admission, however, these records did not have the admission data collection form in which this information is documented.

The policy for Patient Rights and Responsibilities reference #900 A 101 was reviewed. It read under the heading Procedure: All patients will receive a copy of "Patients' Bill of Rights" at the time of admission.
