Based on observation and interview, facility failed to maintain the fire sprinkler system on three of three building levels.

Findings include:

1. Observation on October 13, 2016, at 8:45 AM revealed the fire sprinkler pipe (drain pipe) above the first floor ceiling tile between Admission Registration desk and fire barrier wall had a large load of data lines laying on top of the pipe.

Interview with Senior Maintenance Specialist on October 13, 2016, at 8:45 AM confirmed the data wires were not lifted off of the fire sprinkler pipe.

2. Observation on October 13, 2016, from 10:22 AM to 10:25 AM revealed the sprinkler pipes had data wires draped over the sprinkler pipes in the following locations:
A. Third Floor corridor by room # 325. (10:22 AM)
B. Third Floor corridor by room # 306. (10:25 AM)

Interview with the Senior Maintenance Specialist 2 on October 13, 2016 at 10:25 AM confirmed the sprinkler pipes were draped with data lines.

3. Observation on October 13, 2016, at 10:45 AM revealed the second floor Total Joint Supply Room had a gap greater than 1/8", between the lay-in ceiling and the sprinkler.

Interview with the Senior Maintenance Specialist 2 on October 13, 2016 at 10:45 AM confirmed the gap between the ceiling and sprinkler existed.

Based on observation and interview, facility failed to maintain the electrical equipment on two of three building levels.

Findings include:

1. Observation on October 13, 2016, between 8:55 AM and 10:00 AM revealed the following electrical deficiencies:
A. First floor Physical Therapy hydrocollator was not plugged into a ground fault circuit interrupter. (8:55 AM)
B. First floor Cardiopulmonary reception area had a copier plugged into a surge protector. (10:00 AM)

Interview with Senior Maintenance Specialist on October 13, 2016, at 10:00 AM confirmed the above electrical deficiencies.

2. Observation on October 13, 2016 at 11:10 AM revealed the second floor, room # 204 had an electrical outlet behind the bed that was not fully secured to the outlet box.

Interview with the Senior Maintenance Specialist 2 on October 13, 2016 at 11:10 AM confirmed the electrical outlet was not secured.
