Based on observation, interview, and record review the facility failed to ensure the privacy of patient's were protected when patient's full first and last name was posted in public view in all rooms.
The census was 33.
Observation on 05/24/10 at approximately 3:30 p.m. on the medical-surgical floor all patient room's had erase boards approximately 2X3 feet with patient's full first name and full last name. The erase board was on the wall of the patient's semi-private rooms which would be visable to any visitor.

Based on observation, interview, and record review the facility failed to ensure the patient care responsibilities of proper perineal care of three patient's that had a foley catheter in place.
The censue was 33.
Observation of Staff J, Certified Nurse Assistant, on 05/25/10 at 10:30 a.m. showed Staff J did not dry Patient 10 perineal (lower trunk, groin, and between legs) area with a foley catheter (tube to drain urine out of the urinary bladder) after cleansing with soap and water, then rinsing with clear water. Staff J failed to change gloves between cleansing the perineal area and dressing Patient 10 in clean under clothing.
Observation on 05/25/10 at 10:40 a.m. showed Staff K, Certified Nurse Assistant failed to dry Patient 13 off after preforming perineal care using soap and water, then rinsing with clear water with a foley catheter in place. Staff K then went on and applied moisture barrier cream to the wet perineal area. Staff K failed to change gloves between applying moisture barrier cream and dressing Patient 13 in clean gown.
Observation on 05/25/10 at 11:20 a.m. showed Staff L, Certified Nurse Assistant, failed to dry Patient 2 after preforming perineal care with soap and water then rinsing with clear water with a foley catheter in place. Staff L turned faucet off with clean hands with handwashing.
Record review of Catheter Care #4. showed to rinse and dry area.
Record review of Handwashing: Routine and Scrub showed #7 Turn off faucet, using paper towels for hand-controlled faucet.
Record review of Gloves showed #9 Wash hands after removing gloves. Gloves do not replace hand washing.
