Based on the review of the Emergency Log, the facility failed to maintain a comprehensive log that is accurate, complete and easily retrievable.

Findings include:

The electronic ED Log presented on 03/21/11 at 12:40 did not include patients evaluated in the Labor and Delivery triage area. A separate hand written log for L&D triage was presented at 16:00. The review of the log on 03/22/11 noted gaps in entries and missing information on the disposition of patients. The disposition of 8 patients evaluated on 02/01/11 was not documented. On 02/07/11, the disposition of 9 patients was omitted and on 02/15/11, the disposition of 10 patients were not documented. Similar omissions were noted through out the log.
