Based on observation, it was determined that the facility failed to ensure there was no impediment to the closing of corridor doors.

Findings include:

On 1/30/14 at 9:35 AM, in the presence of Staff #35, the Children's Hospital electrical room door would not close due to an uneven floor.

Based on observation, it was determined that the facility failed to ensure widths of exits are at least 4 feet.

Findings include:

On 1/31/14 at 9:40 AM, in the presence of Staff #35, the northeast Emergency Department exit corridor was reduced to 2 feet wide by a wheelchair, trash can and chair.

Based on observation, it was determined that the facility failed to ensure exit signs are continuously illuminated.

On 1/31/14 at 3:45 PM, in the presence of Staff #35, the exit light at the Same Day Surgery corridor was not lit.
