Facility failed to maintain fire sprinkler in accordance with NFPA 13. Findings included the following:
Surveyor observed between 1:30 and 2:30 pm on 5/05/15 that sprinkler system was "yellow tagged" during inspection of 3/15 and associated documentation indicated that a number of sprinkler heads were needing to be replace.
