Based on observation and staff interview, it was determined that the facility failed to ensure that all means of egress and exits are properly illuminated for use by building occupants at all times.
Findings include:
On 12/3/14 at 11:00 AM, unit 1 Hussey was toured in the presence of Staff #64 and Staff #65. The overhead exterior light fixture had been removed from the wall and not replaced on Exit Door #31.
