Exit interview with the Facilities Manager on May 03, 2022, between 10:45 a.m., and 11:00 a.m., confirmed the common wall deficiency.

Building Construction Type and Height

Based on observation and interview, it was determined the facility failed to maintain building construction requirements in one location, affecting one of four floors.

Findings include:

1. Observation on May 03, 2022, at 10:12 a.m., revealed a piece of structural angle steel, lacked fireproofing at the first floor, 1928 entrance.

Exit Signage

Based on observation and interview, it was determined the facility failed to install exit signage in one location, affecting one of four floors.

Findings include:

1. Observation on May 02, 2022, at 11:47 a.m., revealed exit signage lacking at the bottom of the ramp, entering Radiology.

Exit interview with the Facilities Manager on May 03, 2022, between 10:45 a.m., and 11:00 a.m., confirmed the exit signage deficiency.

Vertical Openings - Enclosure

Based on observation and interview, it was determined the facility failed to maintain vertical openings in three locations, affecting two of four floors.

Findings include:

1. Observation on May 02, 2022, between 10:24 a.m., and May 03, 2022, at 10:05 a.m., revealed the following:

a. 10:24 a.m., on May 02, 2022, a hole within the third floor, expansion joint, located closest to the Elevator C.
b. 11:05 a.m., on May 02, 2022, a floor slab penetration around the supply and return air duct work, located within third floor, Staff Lounge Restroom.
c. 10:05 a.m., on May 03, 2022, a horizontal penetration of the first floor portion of the elevator A enclosure, located within.

Exit interview with the Facilities Manager on May 03, 2022, between 10:45 a.m., and 11:00 a.m., confirmed the vertical opening deficiencies.

Hazardous Areas - Enclosure

Based on observation and interview, it was determined the facility failed to maintain one hazardous area enclosure, affecting one of four floors.

Findings include:

1. Observation on May 02, 2022, at 11:08 a.m., revealed the East Wing, EVS Closet, lacked a self-closing device.

Exit interview with the Facilities Manager on May 03, 2022, between 10:45 a.m., and 11:00 a.m., confirmed the hazardous area enclosure deficiency.

Sprinkler System - Maintenance and Testing

Based on observation and interview, it was determined the facility failed to maintain the automatic sprinkler system in three locations, affecting two of four floors.

Findings include:

1. Observation on May 02, 2022, between 11:01 a.m., and 11:59 a.m., revealed the following:

a. 11:01 a.m., a penetration of the ceiling assembly within the third floor, Multipurpose Room.
b. 11:06 a.m., a missing ceiling tile within the second floor, IT Closet.
c. 11:59 a.m., a missing escutcheon plate within the Labor and Delivery Room 241 bathroom.

Exit interview with the Facilities Manager on May 03, 2022, between 10:45 a.m., and 11:00 a.m., confirmed the automatic sprinkler system deficiencies.

Portable Fire Extinguishers

Based on observation and interview, it was determined the facility failed to maintain portable fire extinguishers in one location, affecting one of four floors.

Findings include:

1. Observation on May 02, 2022, at 10:00 a.m., revealed the K Type portable fire extinguisher, located within Dietary, lacked a placard.

Exit interview with the Facilities Manager on May 03, 2022, between 10:45 a.m., and 11:00 a.m., confirmed the fire extinguisher deficiency.

Corridor - Doors

Based on observation and interview, it was determined the facility failed to maintain corridor openings, in five locations, affecting three of four floors.

Findings include:

1. Observation on May 02, 2022, between 10:12 a.m., and 11:44 a.m., revealed the following:

a. 10:12 a.m., the distance between the fourth floor, Psychiatric Suite doors exceeded one-eighth-inch.
b. 11:14 a.m., the 311 patient room door was not smoke-tight.
c. 11:16 a.m., the 312 patient room door was not smoke-tight.
d. 11:18 a.m., the 326 patient room door was not smoke-tight.
e. 11:44 a.m., the second floor IT Closet door was not smoke-tight.

Exit interview with the Facilities Manager on May 03, 2022, between 10:45 a.m., and 11:00 a.m., confirmed the corridor opening deficiencies.

Gas and Vacuum Piped Systems - Other

Based on observation and interview, it was determined the facility failed to maintain medical gas piping systems, affecting two of four floors.

Findings include:

1. Observation on May 02, 2022, between 10:28 a.m., and 11:51 a.m., revealed the following:

a. 10:28 a.m., medical gas piping within the second floor, protected passageway, lacked labeling and medical gas piping resided on dissimilar metals.
b. 11:51 a.m., medical gas piping was affixed to wiring at the second floor junction of the 1928 and 1991 buildings.
