Based on observations and staff interviews the facility staff failed to identify potential infection control risk in the Emergency Department.
The Findings Include:
On 11/3/11 during the initial tour of the Emergency Department (ED) with the Director of the ED the following items were noted:
Triage room #1: floor was dirty; wires from the computer were lying unsecured under the desk making it difficult to clean the floor. The computer key boards were dirty.
Triage room #2 (EKG room): equipment stored in the room could not be identified by the staff as to its' purpose, a dirty hospital gown was laying upon a 3 drawer chest, and the floor was dirty.
Hallway near room 21: stretcher was dirty and appeared to have dried blood under the mat and the mat had tears, the Crash cart was dusty and dirty, and the Warming cart was dirty
Room #21: Light cover had brown spots all over it
Ice Machine Room: The floor was dirty with trash behind the trash can near the drain, and the cabinets were dirty and sticky
Room #16: Mat on the stretcher was torn
Room #15: Torn sheets and torn mat on the stretcher, also spider webs observed in the room.
Room #14 had 2 stretchers in the room: Torn mat on stretcher #1, tape on the stretcher side rails, appeared to have dried blood on side of stretcher; stretcher #2 was dirty with tape residue; and the cart had sticky substance on the top
Room #13: Mat on stretcher torn, tears in sheets, and the floor was dirty
In rooms 21, 16, 15, 14 and 13 the cabinet doors had been left open exposing the remaining clean linen and hospital gowns.
Behind the nursing station desk the floor was dirty; Candy and trash were on the floor and dust along the area where the wire and cables for the computers were located.
The Director of the ED stated, "I know you won't believe this but (name of nurse manager) and I just this past week did a walk through and identified many of the same things you noted that need to be corrected."
