Based on observation, interview and document review, the facility failed to ensure confidentiality of patient health information was maintained in the mailroom.
Findings include:
On 6/1/15, at 11:09 a.m. the mail room for the hospital which was located close to public bathrooms and the public cafeteria was not secured.
On 6/2/15, at 11:05 a.m. the mail room was observed unoccupied with the door wide open.
On 6/3/15, at 2:05 p.m. the mail room for the hospital was open and no staff were present. At that time, the director of Health Information Management (HIM)-A was interviewed and stated the mail room could contain patient health information, and verified the room was not secured. In addition to no staff present, there were no security cameras in the vinicity.
The facility policy and procedure on Medical Records Maintenance and Retention Policy dated 2/7/14, included: "medical records will be maintained in a secure and confidential manner".
