Based on review of the Centers for Disease Control and Prevention (CDC) COVID-19 guidelines and interview, it was determined the facility failed to mitigate the risks of COVID-19 in that the facility failed to provide evidence of screening of employees for signs and symptoms of COVID-19 for 18 of 18 (04/24/20 through 05/11/20) days reviewed. The failed practice promoted the transmission of the COVID-19 virus and had the likelihood to affect all patients, staff and visitors. Findings included:
A. Review of the CDC COVID-19 Interim Infection Prevention and Control Recommendations for Patients with Suspected or Confirmed Coronavirus Disease 2019 (COVID-19) in Healthcare Setting updated 04/13/20 showed to screen all healthcare personnel at the beginning of their shift for fever and symptoms consistent with COVID-19. Actively take their temperature and document absence of symptoms consistent with COVID-19. If they are ill, have them keep their cloth face covering or facemask on and leave the workplace.
B. In a telephone interview with the Director of Nursing on 05/13/20 at 11:30 AM, when asked for evidence of screening staff for signs and symptoms of COVID-19, the Director of Nursing stated the facility did not document the screening of staff. The Director of Nursing stated each staff member was screened for signs and symptoms of COVID-19 when they entered the hospital, but documentation was not maintained of the screening process.
