On 12/11/24 at 11:25 AM, a review of P2's medical record for an Emergency Department (ED) visit from 12/7/24 was reviewed. On 12/7/24 at 12:10 AM, P2 presented to the ED with a chief complaint of foot pain. The medical record indicated that P2 was seen by a provider on 12/7/24 at 1:59 AM. P2's medical record lacked evidence of an MSE, or a provider note. This finding was confirmed with Staff (S)18, Nursing Informaticist, on 12/13/24 at 10:44 AM.
