Findings include:
1. Observation on June 10, 2014, at 11:07 a.m. revealed the Staff Lounge corridor door lacked smoke-tight integrity.
Interview with the Facilities Manager at 11:08 a.m. on June 10, 2014 confirmed the corridor opening deficiency.

Based on observation and interview, it was determined the facility failed to maintain alcohol-based hand rub stations (ABHR's) in one location, affecting one of six smoke compartments.
Findings include:
1. Observation on June 10, 2014, at 10:49 a.m. revealed a trash can was positioned beneath an ABHR station within the changing area.
Interview with the Facilities Manager at 10:50 a.m. on June 10, 2014, confirmed a combustible trash can was positioned under an ABHR dispenser.
