Based on document review and interview the facility failed to provide the family member (F1) with a written notice of its decision that contains the name of the hospital contact person, the steps taken on behalf of the F1 to investigate the grievance, and the results of the grievance process, and the date of completion in one instance.
Findings include:
1. Review of facility policy, COMPLAINTS & GRIEVANCES, Revised 5/2017, indicated, page 2, C. Feedback to Complaint 4. The Patient Rights Advocate will follow up with a findings letter within 7 days of receipt of complaint.
2. Review of facility documentation indicated the following, on 10/5/2017, patient's family member (F1) complained. F1 indicated patient walked in on 9/29/2017. On 10/5/2017, patient was unable to walk, hard to rouse, and eating very little. F1 requested to have patient sent to another hospital. Patient was sent to another hospital.
3. Interview on 11/9/2017, at approximately 5:15 pm with N1 (Director of Nursing) confirmed findings letter was not sent. N1 confirmed per policy, letter should have been sent.
