Based on observation, it was determined that the facility failed to provide a suitable means to keep a corridor door closed.

Findings include:

1. On 10/31/11 at 10:30 AM in the presence of Staff #5, the corridor door to the main storeroom did not latch.

Based on observation, it was determined that the facility failed to provide a smoke resisting door in a hazardous area.

Findings include:

1. On 10/31/11 at 10:35 AM in the presence of Staff #5, a shaft door was held open with a string in the main store room.

Based on observation, it was determined that the facility failed to ensure the sprinkler system was continuously maintained.

Findings include:

1. On 10/31/11 at 11:15 AM in the presence of Staff #5, five boxes were stored within 18" of the sprinkler in the OR clean supply room.

Based on observation, it was determined that the facility failed to protect all medical gases.

Findings include:

1. On 10/31/11 at 12:25 PM in the presence of Staff #5, an oxygen tank was unsecured in the C-section sub-sterile room.

Based on observation, it was determined that the facility failed to ensure Alcohol Based Hand Rub dispensers (ABHR) are not installed above ignition sources.

Findings include:

1. On 10/31/11 at 11:45 AM in the presence of Staff #5, a ABHR was installed above an electrical switch in the ICU nurse station.

2. On 10/31/11 at 12:10 PM in the presence of Staff #5, an ABHR was installed above an electrical outlet in LDR #3.
