Findings:
Based on review of records and interview, the hospital failed to provide written notice of its decision, steps taken, results of the investigation and date of completion to 1 of 1 grievance received from a patient (Patient #1) regarding the Epilepsy Monitoring Unit (EMU).
The closure letter, dated March 12, 2009, to Patient #1 did not include the hospitals decision, steps taken, completion date or the results of the investigation.
The "Feedback Summary Report", File ID: 7560, submission date 02/18/09, showed on 05/27/09, "PGS (patient guest services) to write closure letter. On 10/02/09, "Spoke with patient informed her not to call and discuss this situation any further with us as she has legal council. No further action will be taken on the hospital side."
The "Clinical Risk Review Committee" meeting minutes, dated 02/15/10, did not include the results of the investigation or resolution to the patient's grievance.
In an interview with Personnel #2, on the afternoon of 03/09/10, he was asked if the closure letter to Patient #1 showed results of the investigation.
