Based on medical record review and interview, the facility failed to ensure wound care was provided according to physician's orders for 1 of 3 (Patient #1) sampled patients.
The findings included:
Medical record review for Patient #1 revealed an admission date of 5/10/2022 with diagnoses which included Necrotizing Faciitis, Altered Mental Status, Cellulits of Right Lower Leg, and Hypertensive Emergency. The patient underwent surgical excision and debridement of the right lower leg on 5/10/2022.
A physician's order dated 5/13/2022 revealed the patient's right lower leg wound care was to be performed as follows: "Dakins wet to dry, packed onto wound. soak kerlix/gauze with dakins, wring out well and pack into wound. to be covered with dry gauze and secured with tape Dressing to be changed daily by nursing staff..."
Review of the nursing documentation dated 5/13/2022 through 5/30/2022 revealed no documentation wound care was completed on 5/18/22, 5/21/22, 5/27/22, 5/28/22, and 5/29/2022.
In an electronic message received on 6/9/2022 at 5:04 PM, the Vice President of Quality and Risk Management verified wound care was not documented as being performed on those dates.
