Based upon record review and interview, the facility failed to hold fire drills quarterly for each shift under varied conditions at different times of the day for four of four quarters reviewed. This has the potential of affecting staff preparation and experience in providing for the protection of all residents in the event of a fire. Findings are:

On 6/23/2014, between 2:00 P.M. and 4:00 P.M., revealed that the facility fire drills were missing documentation for a drill for the second shift during all four quarters of 2014. Results of the record review were acknowledged by Maintenance Staff A.

Based on observation, the facility did not prohibit the use of relocatable power tap as a substitute for adequate wiring. This practice has the potential of affecting the residents and staff in the facility by the possibility of overloading the electrical circuits in the room. Findings are:

On 6/23/2014 between 2:00 P.M. and 4:00 P.M., observation revealed that a relocatable power taps was located in the dining room and nurse ' s kitchen. On 6/23/2014 between 2:00 P.M. and 4:00 P.M., Maintenance Staff A confirmed these observations during the survey process.
