Based on observation made during the fire safety tour of the facility, and staff interview, the facility failed to maintain interior walls and ceilings to be smoke resistant in accordance with (National Fire Protection Association) NFPA 101 LSC (2012) 8.3.5.1.

The findings included:

On 11-7-19 between 10:15 and 11:55 a.m. with the Maintenance Director it was revealed that at the ICU( Intensive Care Unit) entrance, the 100 Wing entrance, and the cafeteria/ kitchen entrance smoke compartments above ceiling it was found that the fire retardant caulk was missing, not enough or falling out of thru wall penetrations and around EMT pipes.

The findings were acknowledged by the Maintenance Director along with the understanding that this is a facility wide deficiency and that all 6 smoke compartments will be checked and verified for compliance by the facilities personnel to insure the safety of its residents, staff and visitors in the event of a fire or smoke event.
