Surveyor observed between 11:00 and 11:30 pm on 4/28/15 that one of two exits from Laboratory did not have an lighted exit sign posted and therefore exit access was not clearly marked.

Surveyor observed between 1:30 and 2:00 pm on 4/28/15 that the last available testing for electrical tesing of receptacles in patient care areas was in 2012. Receptacles shall be tested at least annually in Patient Care areas as follows (NFPA 99, 1999: 3-3.3.3):
Physical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shall be not less than 4 oz. (test)

Surveyor observed between 10:30 and 11:00 am on 4/28/15 that water was standing on the floor in one corner of the emergency electrical switch gear room (NFPA 99, 1999: 3-4.1.1.6).
