based on observations during the facility walk through inspection of the facility with the administrator and director of plant operations, on 12/17/2014, it was noted that;
(a) The Life Safety Branch electrical panel "LLS1" lacked a circuit directory. There shall be a directory for all circuits powered from this panel and the fire alarm power circuit shall be clearly marked with a red marking as the "fire alarm" circuit.
(b) A fire alarm booster panel was not labeled to indicate the electrical panel and circuit that provided power to this booster panel. Note that it shall also be powered from a Life Safety Branch panel.

Based on discussions with the plant operations director and a review of documentation during the facility walk through survey of the facility on 12/17/2014, it was noted that the main Fire Alarm System panel was "red tagged", because one remote annunciator panel located at the EMS building was not functioning. The facility was providing an ongoing fire watch and the local Fire Marshal was monitoring the building until the fire panel is repaired or replaced. The fire alarm system was tested during this facility walk through inspection on 12/17/2014, and it functioned properly, both audible and visual signal devices activated.

Based on a review of the records and discussions with the director of plant operations, during the facility walk through survey/inspection of the facility on 12/17/2014, it was noted that there was no documentation indicating that the emergency generator was being exercised weekly and tested under load conditions at least every 30 days. The facility computer system maintenance network was down and not functioning during this inspection. Therefore the docuemntation could not be printed out or shown.
