Based on document review and interview, the facility failed to provide staffing according to nursing staff matrix in the medical ICU on 9 of 18 available staffing days examined.

Findings included:

1. Per staffing matrix indicated 1 nurse to 2 patients (1:2) in the ICU.

2. Review of 18 available daily staffing sheets for the medical ICU indicated: The following days lacked (1:2) ratios, instead on the following dates, the ratio was 1 nurse to 3 patients (1:3), on 02/09/2018, 02/10/2018, 02/12/2018, 02/13/2018, 02/14/2018, 02/16/2018, 03/02/2018, 03/09/2018 and 05/04/2018.
