Based on observation and staff interview the facility's fire resistance rated corridors were not maintained.

The findings include:

1. On the afternoon of 4/9/2014 observation during a life safety tour of the R Building revealed:

A) That the one hour fire/smoke resistance rated corridor located in the vicinity of Room 2019 had an unsealed hole in the wall, and a partially sealed vent.

B) That the two hour fire/smoke resistance rated corridors within this building was not maintained. Specifically:

- in the vicinity of Room R5050A there was an unsealed junction box located over the fire-rated doors.

- in the vicinity of Room R3050A there was a partially sealed pipe penetration over the fire rated doors.

- in the vicinity of Room R30008 the metal conduit cover for a fire alarm wire conduit was missing over the fire rated doors.

- in the vicinity of Room R1108 there was an unsealed junction box located over the fire rated doors, and spackle was used to seal around a duct. Although the duct may have been installed/maintained according to the required UL listing, the surveyor could not observe this because the spackle covered over the metal flanges.

2. On 4/11/2014 at 1:40 PM observation during a life safety tour of the E Building revealed that the two hour fire resistance rated corridor in the vicinity of Room E5W04 had a duct with an inverted flange. This created an unsealed hole in the wall.

The above findings were concurrently verified by Staff #1.

Based on observation and staff interview the facility's one hour fire rated construction for hazardous areas was not maintained.

The finding was:

On the morning of 4/9/2014 observation in the Switchgear Room, Room RC032, revealed that there was an row of five unsealed 4" conduit penetrations in one of the walls.

The above finding was concurrently verified by Staff #1.

Based on observation and staff interview the facility's exit stairwell two hour fire resistance rating was not maintained.

The findings include:

On 4/11/2014 at 11:00 AM observation in P Building Emergency Exit Stairwell A revealed that there were unsealed data-wire penetrations over the 2nd floor access door, and the 3rd. floor access door.

The above findings were concurrently verified by Staff #1.
