Based on document review and interview the facility failed to investigate allegations of safety issues in one instance (patient 1's incident note).
Findings include:
1. Review of facility Incident Notes: Incident Description, posted by N9 (Patient Advocate Associate) on 12/10/2019 at 9:55 hours indicated the following. ... Patient (patient 1) stated that he was asked about his order and was told that's not going to be done here. Patient stated he was discussing this with a RN and N16 (Medical Doctor Nephrology) and was told, I can end you now by N16. Patient stated that he replied by telling the doctor that he must be psycho and that he interpreted that as him wanting to kill him.
2. Interview with N9 on 12/11/2019 at approximately 15:08 hours confirmed the following. N9 forwarded the Incident Notes: Incident Description she posted on 12/10/2019 at 9:55 hours to the Unit Manager of Dialysis, Case Manager of Case Managers, Manager of Social Workers and Specialist Medical Staff Quality. N9 received the allegation from patient on 10/30/2019. N9 has received no responses to allegations of N16 statement, I can end you, from any of the above named staff.
3. Interview with N5(Accreditation Regulatory Consultant) on 12/11/2019 at approximately 16:15 hours confirmed the following, N5 had no further information on anybody addressing the allegation by patient 1, of N16 statement, I can end you.
