Based on record review and interview the facility failed to ensure patient rights to appeal of discharge was provided prior to discharge.
The findings include:
1. During the review of sampled Patient #2's closed medical record on 05/11/11 at 10:00 a.m., the following information was found.
Patient #2 was admitted to the facility on 03/23/11 through the emergency room (ER) with an admitting diagnosis of Ventricular tachycardia (a rapid heartbeat).
Patient #2 signed an IMPORTANT MESSAGE FROM MEDICARE ABOUT YOUR RIGHTS (IM) form the day of admission at 5:35 p.m..
The second IM form that instructs the patient on how to appeal their discharge was in the closed medical record unsigned. The patient was discharged home on 03/26/11 after the discharge order was written by the attending hospitalist at 6:40 p.m.
This information was confirmed by the Risk Manager during an interview on this date and at this time.
