Based on observation and interview, the provider failed to maintain proper separation of hazardous areas in one randomly observed storage room (medical record storage room). Findings include:
1. Observation at 1:55 p.m. on 4/2/14 revealed a storage room in the patient care wing. That room was over fifty square feet and was being used to store combustible material. The doors to that room were not provided with door closers. Interview with the maintenance supervisor at the time of observation confirmed that finding.
