Based on documentation review and interview, it was determined the facility failed to maintain the emergency lighting for all emergency battery-operated lights.

Findings include:

1. Review of documentation on November 30, 2016, at 10:00 a.m. revealed the facility lacked documentation verifying that the emergency battery-operated lights were tested annually for the required 90 minutes.

Interview with the Maintenance Director on November 30, 2016, at 10:00 a.m. confirmed the emergency lighting documentation was not available at the time of the survey.
