PATIENT RIGHTS: CARE IN SAFE SETTING

Based on review of facility policy and procedures, medical records (MR), and staff interviews (EMP), it was determined the facility failed to follow nursing care policies and procedures for three of ten medical records reviewed (MR4, MR5 and MR6).

Findings include:

Review of facility 'Patient Information Guide" revealed "Patient's Rights and Responsibilities ... Participation in Care Receive Services that incorporate good quality care and high professional standards that are continually maintained and reviewed."

Review of facility policy, Fall Assessment Guidelines, revised 6/2012, revealed, " A. Fall Risk Assessment. 1. Assess patient for presence of fall risk factors and calculate the level of fall Risk: Morse Fall Scale Interventions chart >51... Implement Fall risk Identification System Yellow band on patient's wrist."

1. Review of MR4 on July 15, 2015 revealed the patient had a Morse Fall Scale of 75. Observation of the patient revealed the fall risk bracelet was not on the patient, as per facility policy.
2. Review of MR5 on July 15, 2015 revealed the patient had a Morse Fall Scale of 70. Observation of the patient revealed the fall risk bracelet was not on the patient, as per facility policy.
3. Review of MR6 on July 15, 2015 revealed the patient had a Morse Fall Scale of 60. Observation of the patient revealed the fall risk bracelet was not on the patient, as per facility policy.
During the noted observations, EMP2 confirmed that the facility failed to follow nursing care policies and implement the falls risk identification system wrist bands.
