Based on observation and staff interview, the facility failed to maintain the physical plant in a manner that the safety and well-being of the patients was assured. Findings include:
During observations of the Psychiatric Adult Unit on 6/12 and 6/13/12, the surveyor observed the walls below the skylight located above the nurse's station. The walls below the skylight had several areas where the paint was bubbling away from the wall. One corner had an area approximately 1 foot by 1/2 foot that the plaster and paint was pulling away from the wall.
On 6/12/12 at 3:00 p.m., staff members A and B, the unit director and the unit manager, stated when there was a maintenance issue, the staff would notify or leave a message for staff member M, the administrative coordinator supervisor. The nursing staff would then document the maintenance issue in the maintenance book on the unit. When staff member M receives a work order number, she calls the unit to let them know, and the staff documents the information in the unit book. Staff members A and B stated the staff had put a work order in for the leaking ceiling or skylight. Staff member B stated the roof was flat and there were issues with leaks after the snow melted, or during a heavy rain. In the past few months, the facility had laid a new roof and thought the leaks had been repaired. Then a storm came through and the skylight leaked.
On 6/12/12 at 3:34 p.m., staff member M provided the surveyor with the work order for the leaking ceiling by the adult nursing station dated 1/19/2012. Staff member M stated the work order would deal with the leaking skylight and the walls below the skylight. Staff member M stated due to the skylight leaking and the walls not repaired, the work order could not be marked as done.
On 6/12/12 at 4:30 p.m., staff member O, one of the maintenance and environmental services managers, stated maintenance was aware of the leaking skylight and the issues on the wall below the skylight. Staff member O stated the facility thought after putting a new roof on the building the leak was fixed, but the skylight leaked with the next storm.
The facility has contractors coming to look at the skylight to see what can be done to repair the leaks. Once the skylight leak was fixed, maintenance would repair the wall below the skylight.
