NURSING CARE PLAN

Based on medical record review, and interview, the facility failed to keep a current nursing care plan for one (#7) patient of nine patients reviewed.

The findings included:

Medical record review revealed Patient #7 was admitted to the facility on January 23, 2012, with a diagnosis of Pneumonia.

Review of the Physician's Progress Notes for January 24, 2012, revealed a questionable diagnosis of Tuberculosis (TB) versus Malignancy.

Review of the Physician's Orders revealed an order written January 24, 2012, for "Cont (continous) Respiratory Isolation".

Review of Discharge Summary, dated February 1, 2012, revealed the patient remained in Respiratory Isolation, until discharged on February 1, 2012.

Review of the Medical Record and the computerized Nursing Care Plan, for Patient #7, revealed no nursing interventions were implemented related to the patient's Respiratory Isolation.

Interview with the Director of Nursing (DON) on April 23, 2012, at 2:00 p.m., in the DON's office, confirmed Patient #7 was on Respiratory Isolation and this was not addressed on the patient's care plan. Further interview with the DON revealed there was a care plan available in the computer care plan program, which is to be implemented for patient's on Isolation. Further interview with the DON confirmed the Isolation care plan was not implemented for Patient #7.
