PATIENT RIGHTS: CARE IN SAFE SETTING

Based on observation, policy and procedures, and interview with staff (EMP) it was determined the facility failed to ensure that patient's right to care in a safe setting was maintained.

Findings include:

A review of facility policy " Patients' Bill of Rules and Responsibilities last dated August 16, 2017, revealed "A patient has a right to... . 7. Good quality care and high professional standards that are continually maintained and reviewed. ... . 25. Receive care in a safe setting... . "

An observational tour on March 8, 2018, at 2:30PM of the "Ravdin Four Scope Cleaning Room" with EMP1, EMP2, EMP3 and EMP4 revealed the following environmental conditions:

1. The wall vents of the scope cleaning room were heavily coated with a thick gray dust and debris.

2. The equipment tubing of the disinfection machines were coated with a thick gray dust and debris.

3. The sprout of the water faucet contained a greenish white substance. The hot and cold water faucets contained a greenish white substance around the circumference of the base of the faucets.

4. The tray rack containing clean instruments was layered with rust and contained a thick greenish black substance on each shelf of the rack.

5. The counter where the ventilated hood was positioned contained a thick green debris.

6. The rack with clean connecting block equipment used in the disinfection cleaning units were in the direct pathway for accessing the hand sanitizer wall unit used to sanitize staff hands.

An interview conducted on March 8, 2018, at 3:30PM with EMP1, EMP2, EMP3 and EMP4 confirmed the above findings during the observational tour in the Ravdin Four Scope Cleaning Room. In addition, the facility was unable to produce documentation for the cleaning of Ravdin Four Scope Cleaning Room. Further interview revealed the facility was unable to produce documentation that the staff had been trained to clean the area.
