Based on observation and staff interview, the facility failed to ensure doors in 2-hour fire barrier openings would resist the passage of fire. This condition would allow fire to spread between fire barriers.
Findings are:
Observation on 8/27/20, from 1:07 pm to 1:58 pm revealed:
1. The main 90-minute fire doors by the Cafeteria did not positively latch when auto-closed.
2. The 90-minute fire doors by the Radiology did not positively latch when auto-closed.
3. The 90-minute fire doors by the CECOO Office did not positively latch when auto-closed.
4. The 90-minute fire door to the electrical room in Materials Delivery did not positively latch when auto-closed.
In an interview on 8/27/20, from 1:07 pm to 1:58 pm, Maintenance A confirmed the fire doors did not positively latch.
