Findings include:
Observation on December 21, 2021, at 10:37 a.m., revealed the smoke barrier wall at the end of the finance hall had a 1x1 white data wire penetration.
Interview with the maintenance supervisor on December 21, 2021, at 10:37 a.m., confirmed the wire penetrated the smoke barrier wall.

Based on observation and interview, it was determined the facility failed to maintain electrical outlets to comply with Life Safety Code requirements and electrical safety policies in one of over 10 rooms.
Findings include:
Observation on December 21, 2021, at 10:02 a.m., revealed the laundry room washing machine had an electrical outlet that did not have a ground fault circuit interrupter (GFCI) outlet or breaker located within six feet of the sink.
Interview with the maintenance supervisor on December 21, 2021, at 10:02 a.m., confirmed the electrical outlet did not have a GFCI.
