Through observation during the survey August 24, 2010, it was determined that the facility failed to maintain the fire resistive rating of the vertical opening.
During the walk through of the facility with the Maintenance Director, the facility contained two (2) vertical openings at both entrance/egress paths. The building is setup similar to a bi-level structure. Once one enters the building, the person must go up or down. Both of the vertical openings are not protected per section 39.3.1 and 8.2.5.

Through record review and discussions with the staff during the survey, August 24, 2010, it was determined the facility failed to conduct fire drills on each shift quarterly.
During review of the facility records with the Maintenance Director, the facility conducts one (1) fire drill per year. Fire drills occurred in June 2009 and June 2010.
