Based on observation and interview, doors protecting corridor openings were not maintained so that there would be no impediment to closing the doors.
Findings are:
While touring unit 4M with the Facilities Director on 12/3/2012, a cart was observed stored in the hall in front of the smoke doors at 10:45 am. The smoke doors were held open with magnetic locks tied into the fire alarm system, however, the cart would have prevented one leaf from closing if the magnetic locks released. This finding was verified with the Facilities Director at the time of observation.

Based on observation and interview, vertical openings between floors were not maintained with a fire resistance rating of at least one hour.
Findings are:
During a tour of the third floor with the Facilities Director on 12/3/12 at 1:30 pm, the soiled utility room on 3A had unsealed penetrations from a cable box into the ceiling.
This was confirmed with the Facilities Director at the time of observation.

Based on observation and interview, exit access was not maintained in an accessible manner at all times.
Findings are:
During a tour with the Facilities Director on 12/3/12, the exit corridor to the K Wing mechanical space was blocked by a circle of chairs and a Hoyer lift at 2:30 pm.
This was confirmed with the Facilities Director at the time of observation.

Based on observation and interview, the exit access corridors were not maintained in such a manner as to allow for a clear and unobstructed corridor width of 4 feet.
Findings are:
During a tour with the Facilities Director on 12/3/12 at 10:00am, it was observed on both the 4M and 4A medical-surgical wings that computers on wheels, housekeeping carts and other equipment were left on both sides of the corridors, thus not maintaining the 4 foot clear width necessary for egress.
This was confirmed with the Facilities Director at the time of observation.

Based on observation and interview, the fire alarm system was not maintained in such a manner so that an initiation of an alarm would be easily accomplished.
Findings are:
During a tour with the Facilities Director on 12/3/12, 2 chairs were left in the hall in front of, and blocking the pull station on 4A at 11:15am.
This was confirmed with the Facilities Director at the time of observation.

Findings are:
During a tour with the Facilities Director on 12/3/12 at 1:15pm, the portable fire extinguisher in the Pharmacy clean room was blocked by equipment.
