Based on document review and interview the facility failed to follow Covid-19 protocol and document test results, lot number and expiration dates of home test kits for 5 of 5 employees/staff. (N1-N5)

Findings include:

1. The facility policy titled, "COVID-19 Employee Testing", no policy number, Approval Date date 8/11/2023, indicated if an employee utilizes a home testing kit; the employee will need to provide a photograph of the lot number, expiration date and test results. If the employee utilizes a health care provider or testing location; the employee will need to provide a copy of the test results. All testing results should be sent to the Human Resources Department.

2. In Interview on 12/7/23 at 12:00 pm with staff member A2 (Risk Management) identified N1-N5 as the staff members who tested positive for Covid-19.

3. In interview on 12/7/23 at 12:00 pm with staff member A2 (Risk Management) indicated H1 should have had but does not have record of including photo results of test results, lot numbers and expiration dates for staff members N1-N5 who were positive for Covid-19.

4. In interview on 12/7/23 at 5:00 pm with staff member A1 (Market Chief Executive Officer) indicated the facility did not follow its policy for staff members N1-N5 who tested positive for Covid-19 in regards to documentation of test results, lot numbers, and expiration dates of testing kits.
