Based on observation and a tour of the facility, current and accurate records were not kept of the receipt and distribution of all scheduled drugs.

Findings were:

During a tour of the pharmacy on 1-26-11, a half-empty 375 ml bottle of Everclear (190 proof, 95% alcohol) was discovered in the controlled substances cabinet with no documentation to indicate that its usage was being tracked.

The above was confirmed in an interview with the Chief Executive Officer and the Chief Operating Officer in the administrative conference room on the afternoon of 1-26-11.

UNUSABLE DRUGS NOT USED

Based on observation and a tour of the facility, outdated and otherwise unusable drugs were available for patient use.

Findings were:

During a tour of the patient care area on 1-26-11, two of two foil packets of Xopenex inhalation solution (0.31 mg/3 ml) were found opened and available for use but lacked an " opened " date, as per manufacturer ' s instructions. Manufacturer ' s instructions on the foil packaging state, " Once the foil pouch is opened, the vials should be used within two weeks. "

During a tour of the patient care area on 1-26-11, one of two foil packets of Xopenex inhalation solution (0.63 mg/3 ml) were found opened and available for use. " Date foil pouch opened " indicated that the package had been opened 12-28-10. Manufacturer ' s instructions on the foil packaging state, " Once the foil pouch is opened, the vials should be used within two weeks. "

The above was confirmed in an interview with the Chief Executive Officer and the Chief Operating Officer in the administrative conference room on the afternoon of 1-26-11.
