Based on observation during the morning of 10/28/2010, a Class "K" fire extinguisher was not installed in the kitchen for use with the kitchen range hood fire protection system. A placard must be placed conspicuously near this extinguisher that states, "fire protection system shall be activated prior of using fire extinguisher."

Based on observation during the morning of 10/28/2010, The zone (shut-off) valve for the piped medical gas outlets/inlets in the emergency department trauma room are located in the same room. NFPA 99 requires zone valves be located such that a wall/doors intervenes between the valve and the outlets/intlets that it controls.
