Based on observation and record review, the provider failed to meet the minimum construction standards of the Life Safety Code. The building was an unprotected, Type II (000) structure, without a complete automatic sprinkler system. Findings include:
1. Observation at 11:00 a.m. on 5/4/16 revealed the building was a single story, unprotected, Type II (000) structure, without a complete automatic sprinkler system. Document review of previous survey data confirmed that construction type.
This deficiency would have the potential to affect all twenty-five possible patients as well as numerous staff.

Based on observation and record review, the provider failed to maintain a protected path of egress from the second floor to the exterior of the building for two of two second floor stairways that discharged onto the main level. They were not provided with a one hour fire resistive enclosure to the exterior of the building. Findings include:
1. Observation at 9:30 a.m. on 5/4/16 revealed the two second floor stairways discharged onto the main level. Further observation revealed a continuous one hour enclosure was not provided to the exterior of the building. Review of the previous life safety code survey confirmed those findings. Those stairways were part of the original building constructed in 1948.
This deficiency would only affect staff as no patient care is provided in this building.

1. Observation at 9:45 a.m. 5/4/16 revealed the exterior fire escape stairs on the east and west sides of the building were open construction and less than 24 inches in width down to the level of exit discharge. Review of previous survey data revealed the stairs were part of the original building constructed in 1948.
This deficiency would only affect staff as no patient care is provided in this building.
