1. Phone revisit at 9:40 a.m. on 8/26/13 revealed the top of the two hour separation wall between the clinic and the hospital was not sealed at the roof deck. Fire safing insulation was placed in the decking open spaces above the 5/8 inch thick gypsum board on the wall. The safing insulation was not sealed with an approved firestop material. Interview with the administrator during the phone revisit confirmed those findings. He stated there was construction in progress at that location.

2. Phone revisit at 9:45 a.m. on 8/26/13 revealed the clinic business office and the clinic storeroom were not provided with smoke detectors interconnected to the fire alarm system. The storeroom also was not sprinklered. Interview with the administrator during the phone revisit revealed future construction work was planned in the area. The corrections had not been done yet.
