Based on record review and interview, the hospital failed to ensure pharmaceutical services were administered according to accepted professional principles as evidenced by not having all prescriber's orders reviewed for appropriateness by a pharmacist before the first dose was dispensed. Findings:

Review of the plan of correction from the survey conducted on 07/10/14 revealed the hospital currently does not review prescriber's orders for appropriateness after hours by a pharmacist before the 1st dose is dispensed.

SF8Pharmacy presented 2 documents on 08/19/14 at 11:45 a.m. which she indicated as being copies of the proposal to a Hospital Pharmacy to provide after hours pharmacy services.

Review of the document titled : "Telepharmacy Activities" reads in part: "We defined telepharmacy broadly to include a range of activities such as having a small rural hospital fax or electronically transmit medication orders for review by a pharmacist at another hospital. (The name of the Hospital ) will verify orders daily starting at 6pm."

In an interview on 08/19/14 at 11:50 a.m., SF8Pharmacy indicated the hospital was in negotiations with a Pharmacy to provide services for 1st dose review on medications ordered after hours. SF8Pharmacy indicated that 1st dose review was currently not being performed for medications ordered after hours and would not be until a remote pharmacy was in place.
