DISCHARGE PLANNING - EARLY IDENTIFICATION

Based on medical record review, interview, and policy review the facility failed to ensure that discharge planning assessments were completed in a timely manner for 3 (#1, #8, & #9) out of 10 patients currently hospitalized resulting in the potential for poor patient outcomes. Findings include:

On 7/23/14 at approximately 1050 during an interview and review of patient #8's medical record revealed the patient was admitted to the facility on 7/18/14 and at the time of review no initial discharge plan assessment had been completed. Staff C confirmed this finding at the time of observation stating "There is no discharge plan in here as of yet." Staff C was queried as to when discharge planning commences for patients to which she replied, "Within 48 hours (of admission) the initial discharge plan is started." Staff C was queried as to if a discharge planning assessment should be in place at this time to which she replied, "Yeah."

On 7/23/14 at approximately 1114 a review of patient #9's medical record revealed the patient was admitted to the facility on 7/12/14 and the initial discharge planning assessment was completed on 7/17/14. On 7/23/14 at approximately 1114 Staff C was queried as to if the discharge plan was completed in the aforementioned time frame to which she replied, "No."

On 7/23/14 at approximately 1300 a review of the policy titled, "Discharge Plan" dated 1/2/13 revealed under #5 "Case manager will implement an initial discharge planning assessment on all patients within 2 business days of admission....".

On 7/23/14 at approximately 1030 during medical record review of patient #1 it was revealed that the patient was admitted to the hospital on 7/18/14 and at the time of review there was no initial discharge plan evaluation completed for this patient. This finding was confirmed on 7/23/14 at approximately 1030 by staff C.
